Your 401(k) online portal has to be the single worst web application I've seen in a great many years of dealing with web applications. As near as I can tell, your applications developers have never ever used a web application in their lives. I can tell this by the way your web application does all sorts of incredibly annoying things that are exactly opposite the way everyone else in the world does it. For instance - showing ever error message by showing an html page whose only purpose is to popup a javascript alert and then redirect the user? Bad bad bad!
Also - does anyone out there really believe that having me select an image when I go to log in, in addition to remember my unique user name (which must contain a number, for some absolutely bizarre reason) as well as a password is in any way adding to the security of your site?
In short, if your application developers can't deal with this (probably because you've under-funded them for years, and are loading them down with all sorts of other requests, or they may even be incompetent, I don't know.), please hire someone to do your site intelligently.
